State Youth Council Application

Thank you for your interest in the Georgia National Guard State Youth Council!  The State Youth Council consists of volunteer members between the ages of 13-17 years old.  The Junior State Youth Council consists of members between the ages of 9-12 years old.  In-person meetings are held four times yearly to train, brainstorm, and plan activities that serve to enhance the quality of life for our Guard Youth.  Other meetings are held on the zoom platform throughout the year.
The goal of the Georgia National Guard State Youth Council is for Guard Youth to work together in order to identify and define issues that specifically affect them. Our goal is to actively engage Guard Youth in addressing and implementing ways to foster personal growth and enhance the lives of military youth.
Please answer and complete all items in this application.  After review of all returned application packets, you will be notified by email of the decision. Please be sure you complete and return all items in order for your application to be processed. If you have any questions regarding the application process, please contact the Child & Youth Director.

Qualifications
  • Must be between the ages of 13-17 (SYC) or 9-12 (JR SYC)
  • Maintain high academic standing at school (3.0 GPA or higher)   *If your GPA is lower than a 3.0, please fill out application, and we will work with you on a plan to raise your GPA*
  • Ability to attend quarterly meetings, webinars, and/or conference calls
  • Must be willing to serve at least one year
  • Willingness and desire to work with other military youth
  • Must represent GANG Youth Council at statewide and community events in a professional and positive manner
  • Strong written and oral communication skills
  • Ability to work as a team player and actively participate

    To ensure that the Georgia National Guard State Youth Council is a positive and enjoyable experience for all participants, it is necessary to establish and enforce high standards of behavior. Please read the following and sign below.

    I, a representative of the GANG State Youth Council, will uphold the following conduct and behavior standards:
    • I will be courteous and respectful towards others.
    • I agree to value and respect others’ ideas, regardless whether they are the same as my own.
    • I will actively participate in all activities during training sessions.
    • I will conduct myself in a professional manner at all time.
    • I will dress appropriately at all times. Revealing clothing or apparel featuring alcohol, tobacco, and other drug messages are prohibited.
    • I will not use any alcohol, tobacco, or other drugs, or engage in any behavior of a sexual or violent nature at any time during the trainings.
    • I understand that I will forfeit my position as a GANG State Youth Council member for any misconduct or repeated behavior. ​

    As a representative of the Georgia National Guard Child and Youth Program, I represent not only myself, but the National Guard children and youth throughout Georgia, and I pledge to uphold this commitment. I understand that if I am not able to remain in good standing by attending meetings and activities, I will be asked to leave.
    I have witnessed the pledge made my son/daughter and will support him/her in carrying out the State Youth Council duties. I understand that if my son/daughter breaks any of the commitments stated in this Code of Conduct, they will be dismissed from the GANG State Youth Council. 

    Section V:  State Youth Council Release

    I have reviewed, understand, and agree to the terms of this agreement. I will support my child as he/she supports the National Guard Youth Program. I am willing to allow my child to travel to fulfill all responsibilities of this appointment. In addition, I hold the National Guard Bureau, Family Programs staff, and the National Guard Youth Program harmless from injury or death that could occur as a result of participation in any activity. 

    Section VI: Photo Release

    I hereby grant the Georgia National Guard the right and permission to use and publish the photographs/video material taken at youth events in order to develop photographic and multimedia materials. These materials may be used to advertise, market, and promote the Youth Program. I understand that identifying information (e.g. name, address, or city) will not be used in this media without my permission.

    Section VII:  Medical Release

    If your child has allergies, medication needs, or any other medical condition we need to be aware of, please complete the information below. Include all prescription and/or over-the- counter medication. When the youth representative attends symposiums and/or other events all listed material must be in its original container and you must include any items (inhalers, spoons, cups, etc.) needed to properly dispense the medication.

    Authorization to Self-Medicate

    If your child typically self-medicates, please indicate your permission for him/her to do so while attending the Georgia National Guard Youth Program events.

    Authorization to Treat

    I, the parent/legal guardian of the State Youth Council Applicant Above, authorize and consent to medical, surgical, hospital care, treatment and procedures to be deemed immediately necessary or advisable by a physician to safeguard my child’s health during Georgia National Guard Youth Programs. I waive my rights of informed consent to such treatment. I also authorize a copy of this consent to be treated with the same authority as the original one.

    Section VIII:  Consent to Participate

    Youth Consent

    I have read and understand all of the requirements for membership of the State Youth Council. I am prepared to accept responsibility of active membership and the obligations required to fulfill the duties of my membership. 

    Parental Consent


    I hereby give my consent for my child to seek the voluntary position of representative on the Georgia National Guard State Youth Council. I have read and understand all of the requirements for membership of the State Youth Council.
    I give my permission for the following information for my youth to be shared with fellow State Youth Council Members:

    -Telephone Number
    -Email 

    I understand that transportation to and from events will be the responsibility of the delegate or guardian.

    Section IX:   State Youth Council Recommendation Form

    Applicant: Please include the information of a trusted adult not related to you, who you think can speak to your strengths (e.g. teacher, coach, principal, or counselor).  By including this person's information you have approved and consented with them that we may contact them by email within the next five days for a recommendation statement.
